2023/24 PE Field Trips - Cornerstone Christian Education

Throughout the school year, PE classes may visit Jensen Park Playground (305 1 Ave SE) for various activities to support the curriculum. School policy requires the school to have the consent of parents each time the student leaves the school grounds. We will send home a permission slip for every class field trip except for the PE Class. Instead this form will serve as a blanket form for all PE field trips to Jensen Park.

School Responsibilities
The school will make every reasonable effort to ensure or ascertain that:

• There is adequate staff or volunteers.
• Staff and volunteers are suitably trained and qualified.
• The students are adequately supervised during the activity/program.
• The location used are appropriate and safe for the activity and group.
• The location used will require staff, students, and volunteers to walk,
• The equipment used has been inspected and deemed appropriate and safe.
• Students will not be transported by vehicles.

Consent and Acknowledgement of Risk:
Parents have the right to obtain as much information as they require about these field trips and associated risks and hazards, including information beyond that provided to them by the school. Parents should realize that there are risks and/or hazards inherent in any field trip and acknowledge that their child may suffer personal and potentially serious injury due to an unforeseeable event associated with his/her participation. Potential known hazards include but are not limited to the following: injuries related to falls, sporting equipment /use, insects, animals, water, mud, and other outdoor hazards.

Parents are responsible to inform their child that he/she is to abide by the rules and regulations, including directions and instructions from the volunteers, administrators, teachers, and supervisors over all phases of the program/activity.
In the event that your child fails to abide by these rules and regulations, disciplinary action may require his/her exclusion from further participation, or that the parents be contacted to have him/her picked up. It is the parent’s responsibility to advise the school of any medical and/or health concerns of their child, which may affect his/her participation in the stated field trips.

Methods of Transportation:
Students will be required to walk to Jensen Park.

I understand that in the event of a serious accident or illness, I am giving permission for my child to be transported to the nearest emergency medical facility, and that I am responsible for all expenses. Students are reminded that all school rules are in effect during the field trip,
